I agree to the following terms and accept responsibility for myself and, if qualified, I also accept responsibility for my guest.

CODE OF CONDUCT
By signing this document I recognize that violations of the code of conduct policy may result in immediate actions including; sending me and my guests home immediately, written warning on permanent record, or termination. I am responsible for and will compensate the Company for any and all damages I inflict on resort property either by accident, improper conduct, or negligence.

The following are unacceptable behaviors:

-Theft or inappropriate removal or possession of property.
-Possession, manufacture, sale, transfer, distribution or use of illegal drugs.
-Fighting or threatening violence.
-Immoral actions or intimidating others.
-Boisterous or disruptive activity.
-Negligence or improper conduct leading to damage of resort property.
-Violation of reasonable safety and health rules.
-Sexual or other unlawful or unwelcome harassment or touching.
-Sexual relations between co-workers.

By signing this agreement I pledge to uphold the professionalism, image and values of Accord Group and follow all guidelines stated. I will maintain a clean and safe living space. I will follow all regulations imposed by resort and I will respect neighboring guests and make every effort to be a great guest.

WAIVER OF LIABILITY and HOLD HARMLESS
I hereby fully waive and release Accord Marketing and Management (the ‘Releasee’), from any and all claims for personal injury, property damage, or death that may result from my participation in the following activities:
the Annual Awards Banquet and the Trip (cruise):

I hereby voluntarily, at my own risk, sign this Waiver and Assumption of Risk.
I hereby acknowledge and understand that there are dangers and risks associated with the activities described above, which have been fully explained to me.
By signing this Waiver and Assumption of Risk, I fully assume the dangers and risks, and agree to use my best judgment while engaging in those activities. I further agree to indemnify and hold harmless the Releasee, its employees, agents, officers, from and against any and all liability incurred as a result of or in any manner related to my participation in the activities.

I hereby certify that I am of legal age and competent to execute this Waiver and Assumption of Risk, that in doing so of my own free will and accord, voluntarily and without duress, and that I do so intending to bind myself, my executor, my heirs, and administrators or assigns to the fullest extent.

I have read and understood the foregoing, and acknowledge my consent to the terms of this Waiver & Assumption of Risk by signing this Waiver.
